Head Start 2022-2023

If you are interested in registering for our ACE Prep Academy formally known as the Head Start program, please use the provided link. Be sure to add all household members and upload the required documents before submitting them.

Required Documents:

  1. Proof of Income: 2 Pay Stubs, W-2, 1040 Tax Form, or Statement from Employer.
  2. Proof of Child’s Identity: Child’s birth certificate, notification of birth from the hospital, or child’s identity from a child-placing agency (foster care/adoption agencies).
  3. Proof of Residence: Utility Bill (electric, water, gas, etc.), Lease/Mortgage/Rental document.
  4. Current Signed Physical; Immunization (Shot) Records and Dental Exam.
  5. Two Emergency Contacts Including Addresses and Phone Numbers.
  6. Custody of Court Documents (if applicable).

We cannot consider any application until Head Start Family Services staff has reviewed this information. Thank you.

Documentacion Requerida:

  2. Prueba de Identidad del nino(a): Una partida de nacimiento certificada, pasaporte, ortra prueba de identidad del nino(a) de una agencia de adopcion, hogares sustitutos o agencias de adaptacion).
  3. Prueba de direccion: Cuenta o recibo de electricidad, agua, gas o prueba de hipoteca o arredramiento.
  4. Examen medico general y registro o certificado de vacunas al dia, examen dental.
  5. Dos contactos de emergencia incluyendo direccion y numero de telefono.
  6. Custodia legal del nino(a) o documentos judiciales (si aplica).
